Any written consent required to be appended to the application shall be in the prescribed form but need not be in duplicate.

Except in a case where the applicant has made a previous application to a Court, or the Court requires the attendance of the applicant, the attendance in person of the applicant shall not be necessary for the purpose of lodging the application and documents.
