(2)
A registered practitioner shall make an application for a practising certificate to the Registrar in the prescribed form and pay the prescribed fee.

A practising certificate shall be valid for not more than twelve months from the date of issuance and shall contain a unique alpha-numerical reference number.
(4)
The Registrar shall maintain a register of registered practitioners who have been issued with practising certificates.
(5)
All prescriptions, reports or referrals produced, made or issued by a registered practitioner to a patient or any third party as representative of the patient shall bear the signature of the registered practitioner and the registration number of his practising certificate.
(6)
The application for the renewal of an annual practising certificate shall be made to the Registrar not less than thirty days before the date of expiry in the prescribed form together with payment of the prescribed fee.
(7)
All practising certificates shall be displayed in such manner as may be prescribed at the registered practitioner’s place of practice.
Revocation of registration
