Application shall be made on the prescribed application forms obtainable from any offices as follows:
(i)
State RISDA Office of Malacca;
(ii)
RISDA District Office of Jasin, Alor Gajah and Melaka Tengah; and
(iii)
Field Staff of the Rubber Industry Smallholders Development Authority
(RISDA).
