Permohonan-permohonan hendaklah dibuat di atas borang-borang yang ditentukan yang boleh didapati daripada mana-mana berikut:

Darul Aman.

(ii)

Pejabat RISDA Daerah:

Kota Setar/Kubang Pasu/Pendang/Langkawi

Kuala Muda/Yan, Sungai Petani

(d)

Sik

(e)

Kulim/Bandar Baharu, Kulim

(iii)

Pejabat RISDA Stesen diseluruh Negeri Kedah Darul Aman.

Permohonan yang tidak dibuat di atas borang-borang yang ditentukan atau tidak diterima dalam tempoh yang dinyatakan di atas atau yang tidak dibuat terhadap kebun kecil yang didaftarkan di bawah perenggan 7 Skim itu akan dianggap sebagai permohonan-permohonan yang tidak dilayani.

I, Sopian bin Abu Bakar, State RISDA Director of the State of Kedah Darul

Aman in exercise of the powers conferred on me by paragraph 8 of the Rubber

Industry Smallholders Development Authority (Scheme No. 6) Regulations 1981, hereby give notice that the “Register of Applications for Grants for the

Replanting of Rubber Smallholdings” for the year 2012 shall be opened on the 1 January 2012 and shall be closed on the 31 December 2012.

Seksyen 2

Application should be made on the prescribed application forms obtainable from any of the followings:

(i)

State RISDA Office at Jalan Telok Wanjah, 05990 Alor Setar, Kedah

Darul Aman.

(ii)

RISDA District Offices at:

(a)

Kota Setar/Kubang Pasu/Pendang/Langkawi

(b)

Padang Terap, Kuala Nerang

(c)

Kuala Muda/Yan, Sungai Petani

(d)

Sik

(e)

Kulim/Bandar Baharu, Kulim

(f)

Baling

(iii)

Station RISDA Office at The Whole State of Kedah Darul Aman.

(iv)

Field Staff of the Rubber Industry Smallholders Development Authority

(RISDA).

(v)

RISDA Website and Portal myGovernment through the internet.

Seksyen 3

Any application which is not made on the prescribed application forms or is not received within the period prescribed above or is not made in respect of a smallholding registered under paragraph 7 of the Scheme may be considered as an inadmissible application.
