Lesen ini tidak boleh dipindahkan hakmiliknya dan dikeluarkan tertakluk kepada *syarat-syarat yang berikut:
(a)
lesen ini hendaklah dipamerkan pada sepanjang masa di premis yang dinyatakan dalam lesen ini;
(b)
tiada seorang pun boleh menjalankan suatu **syarikat/perniagaan yang mengendalikan kawalan makhluk perosak selain di premis yang dinyatakan dalam lesen ini;
(c)
Lembaga tidak akan mengeluarkan lesen pengendali kawalan makhluk perosak sehingga pemohon, atau seorang pemakai racun makhluk perosak yang bekerja di bawahnya yang menjalankan atau menyelia penjalanan pengendalian kawalan makhluk perosak ke atas harta orang lain bagi upah, ialah pemakai racun makhluk perosak berlesen;
(d)
pemegang lesen hendaklah memohon kepada Lembaga berkenaan dengan apa-apa pertukaran dalam alamat **syarikat/perniagaan yang mengendalikan kawalan makhluk perosak itu sekurang-kurangnya sepuluh hari sebelum pertukaran itu, disertakan dengan satu salinan Lesen untuk Menjual dan
Menyimpan untuk Jualan Racun Makhluk Perosak yang dikeluarkan oleh

Perosak dan, dalam hal perniagaan yang mengendalikan kawalan makhluk perosak, lesen perniagaan yang dikeluarkan oleh pihak berkuasa tempatan yang berkenaan bagi premis yang baru;

pemegang lesen hendaklah memaklumkan kepada Lembaga berkenaan dengan apa-apa perubahan mengenai pemakai racun makhluk perosak berlesen yang bekerja di bawahnya dalam tempoh sepuluh hari daripada perubahan itu.

Preamble
- IN exercise of the powers conferred by paragraph 57(1)(k) of the Pesticides Act 1974 [Act 149], the Minister, after consulting the Board, makes the following rules: Citation 1. These rules may be cited as the Pesticides (Pest Control Operator) (Amendment) Rules 2024. Amendment of Schedule 2. Pesticides (Pest Control Operator) Rules 2004 [P.U. (A) 70/2004] are amended in the Schedule—
(a)
by substituting for Form C the following form:

Unless earlier cancelled or suspended, this licence is valid for a period of two years from the date of its issue.
Seksyen 3
This licence is not transferable and is issued subject to the following
*conditions:
(a)
this licence shall be exhibited at all times at the premises specified in this licence;
(b)
no person shall conduct a pest control operating
**company/business other than at the premises specified in this licence;
(c)
the Board shall not issue a pest control operator licence until the applicant, or a pesticide applicator under his employment who carries out or supervises the carrying out pest control operations on the property of another person for hire, is a licensed pesticide applicator;
(d)
the licensee shall apply to the Board in respect of any change in the address of the pest control operating **company/business at least ten days prior to the change, accompanied with a copy of the Pesticide Sale and Storage for Sale Licence issued by the Pesticide Licensing Officer and, in the case of a pest control operating business, the business licence issued by the respective local authority for the new premises;
(e)
the licensee shall inform the Board in respect of any change in the licensed pesticide applicator under his employment within ten days of the change.

**Conditions:

this licence is granted to the licensee to carry out general house pest control, termite and other wood destroying organisms control, general public weed control and control of pests affecting public health;
Seksyen 2
unless earlier cancelled or suspended, this licence is valid for a period of two years from the date of its issue;
Seksyen 3
this licence is not transferable;
Seksyen 4
this licence shall be worn by the licensee when he carries out pest control operations;
Seksyen 5
if this licence is lost, the licensee shall report the matter to the Board and apply for its replacement;
