SYARAT PERAKUAN KESIHATAN VETERINAR
1.
PERAKUAN KESIHATAN VETERINAR ini adalah bagi binatang, tempoh dan tujuan yang dinyatakan di muka sebelah.
2.
PERAKUAN KESIHATAN VETERINAR ini sahlaku sekali untuk satu tujuan sahaja.
3.
Pemegangnya hendaklah membenarkan pegawai yang diberi kuasa oleh Jabatan Perkhidmatan
Haiwan memeriksa dan menandakan binatang dan bahan binatang ini jika diperlukan.
4.
PERAKUAN KESIHATAN VETERINAR ini hendaklah sentiasa bersama binatang dan bahan binatang berkenaan bagi memudahkan pegawai yang diberi kuasa oleh Jabatan Perkhidmatan
Haiwan untuk memeriksanya.
5.
Kegagalan untuk mendapatkan PERAKUAN KESIHATAN VETERINAR untuk tujuan pindah atau sembelih merupakan satu kesalahan di bawah Ordinan Binatang 1953 dan boleh didakwa di Mahkamah.

In exercise of the powers conferred by section 82 of the Animals Ordinance 1953 [Ordinance No. 17 of 1953], the Menteri Besar of Pahang appoints the following places to be Veterinary Centres in the State of Pahang.

Ternakan Jengka shall be as prescribed in the First Schedule.
2.
A Veterinary Health Certificate as in the Third Schedule shall be issued upon completion of a health examination.
3.
The charges and the conditions for shed and weighing services in Pusat
Pemasaran Ternakan Jengka shall be as prescribed in the Second Schedule.
4.
Federation of Malaya Government Gazette No. 335 published on 29 September 1949 and No. 416 published on 5 November 1953 are revoked.

Double charge shall be imposed on public holidays and after office hours.

A Veterinary Officer, Assistant Veterinary Officer and the Officer in Charge of Veterinary
Centres shall have full discretion to reduce or waive the payment of any fees if he is satisfied that the owner of the animals treated is too poor to pay the fees or charges.
15.
There will be no charges for post-mortem examinations and tests carried out for the purpose of the department.
16.
The attendance of a Veterinary Officer, Assistant Veterinary Officer and the Officer in
Charge of Veterinary Centres at a Veterinary Centre or elsewhere is subject to the exigencies of his other duties and cannot be guaranteed.

RENTAL CONDITIONS
(a) The feeding, security and the cleanliness of the animals and shed is the responsibility of the tenant.
(b) Shed rental is based on first come first serve basis.
(c) The fees shall be paid prior to the arrival of the animals.
(d) The applicant must indicate the number of the animals, compartments and the duration of the keeping of the animals.
(e) The tenant is responsible for the security, health and comfort of their animals.
(f) The tenant must surrender the compartment in a clean and good condition after the animals have been discharged.
(g) Negligent tenants or tenants who are disobedient and fail to comply with the rental conditions will be given less priority in the future.

CONDITIONS OF VETERINARY HEALTH CERTIFICATE
1.
This VETERINARY HEALTH CERTIFICATE is for the animals, period and purpose specified overleaf.
2.
This VETERINARY HEALTH CERTIFICATE is only valid once and for one purpose only.
3.
The holder shall allow the officers authorised by the Veterinary Services Department to inspect and identify the animals and animal product if necessary.
4.
This VETERINARY HEALTH CERTIFICATE shall continuously be kept with the animals and animal product to facilitate inspection by the officers authorised by the Veterinary Services
Department.
5.
Failure to obtain VETERINARY HEALTH CERTIFICATE for the purposes of removal or slaughter of animals constitutes an offence under the Animals Ordinance 1953 and is liable to prosecution in Court.
