Section 5
(1)
A person applying for a licence to carry out any activity under section 4 of the Enactment shall submit the application to the Commission together with the details as specified in section 5 of the Enactment.
(2)
The application under subregulation (1) shall be in Form A as prescribed in the First Schedule.
