(2)
No publication made under subsection (1) shall contain the name of, or any information that could lead to the identification of, any person whose death has been considered by a Committee or any medical practitioner or dental practitioner or any other person who has in any way been involved in the care or treatment of the deceased person.
(3)
Notwithstanding subsections (1) and (2), the Chairman of a
Committee shall furnish to the medical practitioner or dental practitioner whose patient’s death was notified as an assessable death the conclusion and recommendations of the Committee.
