Opening note
Preamble
- IN exercise of the powers conferred by section 42 of the Optical Act 1991 [Act 469], the Minister, after consulting the Malaysian Optical Council, makes the following regulations: Citation 1. These regulations may be cited as the Optical (Amendment) Regulations 2017. Amendment of Schedule 2. The Schedule to the Optical Regulations 1994 [P.U. (A) 210/1994] is amended by substituting for Form 17 the following form: “Form 17 [Regulation 24] OPTICAL ACT 1991 [Section 30] OPTICAL REGULATIONS 1994 PERMIT TO PRESCRIBE AND DISPENSE CONTACT LENS Full Registration Certificate No.: _____________________________ Permit to Prescribe and Dispense Contact Lens No.: __________________________ IT IS HEREBY CERTIFIED THAT ________________________________________________________ (I/c No. / Passport No.: __________________________) residing at __________________________ is a registered optician who is permitted to prescribe and dispense contact lens subject to the renewal of his/her annual practicing certificate as an optician.
