Section 5
—
(1)
The Regional Registrar shall cause to be provided a sufficient number of registers and prescribed forms for the registration of all births, still-births and deaths, and shall furnish to every Senior
Registrar such registers as he requires.
(2)
Such registers shall be in the prescribed form.
(3)
The Regional Registrar shall cause to be provided such additional prescribed forms as are necessary for the carrying out of the provisions of this Ordinance.
