Section 116
(1)
A fee shall be paid for the issuance and renewal of a Certificate of
Approval referred to in subregulations 114(2) and 115(2) at the rate as prescribed in Part II of the Third Schedule.
(2)
If a Certificate of Approval issued by the Commission is lost, stolen, destroyed, mutilated, or defaced, a replacement of the Certificate may be issued

by the Commission on production of a statutory declaration by the holder that the original Certificate has been lost, stolen or destroyed or upon surrender of the mutilated of defaced original Certificate.
(3)
The fee for the replacement certificate issued under subregulation (2) is as prescribed in Part II of the Third Schedule.
