Section 11
—
(1)
Application for registration shall be made to the Board through the Registrar, in such form as may be prescribed from time to time by the Board.
(2)
Without prejudice to the provisions of subsection (1), the form to be prescribed by the Board shall contain a declaration as set out in the First Schedule, to be made in accordance with the Statutory
Declarations Act 1960 [Act 13].
(3)
The Board shall cause to be registered in the Register any person whose application complies with the requirements of this section and who possesses the qualifications for registration stipulated in section 9.
