Section 4
(1)
An application to the Court for an adoption order shall be made by filing with the Registrar of the Court an application in Form 1, verified by the affidavit of the applicant, to which affidavit there shall be exhibited any document referred to in the application.

The proposed adopter shall be the applicant and the persons mentioned in rule 10 shall be the respondents.
(3)
Save as provided in rule 10, no person shall be served with a copy of the application.
(4)
The notice to be served on every respondent shall be in Form 2 and a copy shall be served on the guardian ad litem.
