(a)
in the prescribed form and manner;
(b)
accompanied by the prescribed fee; and

(c)
by submitting together with the application—
(i)
the approval letter; and
(ii)
any information, particulars and documents as may be specified by the Director General.
(2)
At any time after receiving the application, the Director
General may by a written notice require the applicant to provide additional information, particulars or documents.
(3)
Where any additional information, particulars or documents required under subsection (2) is not provided by the applicant within the period specified in the notice or any extension of the time as may be granted by the Director General, the application shall be deemed to be withdrawn and shall not be further proceeded with, without prejudice to a fresh application being made by the applicant.
(4)
An application for a licence may be withdrawn by a written notice to the Director General at any time before the application is granted or refused.
